From the 2010 rulebook
If the attacking unit has a second CC weapon, he may move a second Inch and make a second attack; if the opponent survives and also has a second CC weapon, he can then make a second Counter.
So if I get this right then if John with two hand weapons fights Betty who only has a heavy weapon, that on the second attack that John makes on his turn Betty does not get a counter.

IMO this should be stated again under 5.2 Close combat, Countering, Counter attacking.
